Portfolio Forms and Instructions

The 4-H portfolio is an opportunity for 4-H members to record and analyze their learning experiences in the 4-H program.  Members who submit portfolios for judging receive evaluation and recognition for their accomplishments.  Portfolios are due on October 1 each year.  The forms and instructions linked below will provide the basis for completing 4-H portfolios.  Please contact the Extension Office if you would like a copy of the forms in Word so you can complete your portfolio electronically.

4-H'ers should complete one member record form and include it in their project portfolio.

Junior (ages 9-11), Intermediate (ages 12-13), and Senior (ages 14-19) 4-H'ers should complete one project record form for each project they complete.

Cloverbuds may also submit project portfolios although they will just be provided with comments and will receive participation ribbons.

4H'ers who have worked on a project for more than one year may use this form to record information about past learning experiences.
